What is a medical home?

A medical home provides comprehensive, coordinated, culturally competent, family-centered, community based care. From the National Center for Medical Home Implementation:

A family-centered medical home is not a building, house, hospital, or home healthcare service, but rather an approach to providing comprehensive primary care.

In a family-centered medical home the pediatric care team works in partnership with a child and a child’s family to assure that all of the medical and non-medical needs of the patient are met.

At Family Voices Colorado, we believe that every child and youth deserves a medical home.  But it’s especially important for children with special health care needs, who require more care or services than typical kids. Practices certified as medical homes are committed to making sure patients and families are partners in decision-making, and they go the extra mile in helping families access, understand, and coordinate care, from surgery to occupational therapy; from oral health to mental health; from medication to augmentative communication devices.

Family Voices is committed to supporting practices that are or want to become certified as medical homes at every step of the process, from conducting the Medical Home Index and Family Surveys to Quality Improvement projects and ongoing support. We help providers better understand and navigate insurance or disability systems on behalf of patients and families. We also help families who are looking for providers certified as medical homes, especially those that accept Medicaid and CHP+.
